Insurance Requirements

You must have a Certificate of Insurance (PDF) for general liability in the amount of $1,000,000, naming City of Madison as "Additional Insured," before a license can be issued. Insurance may be purchased from a private insurance provider.

Your insurance certificate must bear the stamped approval of the Risk Manager before your craft vendor application can be reviewed. Please add your name to your insurance certificate for identification purposes.
