  • Excavate in the Right-of-Way, download and fill-in PDF, send via POSTAL MAIL
  • Excavate in the Right-of-Way, send via EMAIL

    Follow these steps to email the completed form:

    1. Save the completed form.
    2. Open your email application & attach the completed saved form to the email.
    3. Email the completed form to chemenway@cityofmadison.com.

    Please note you must also attach an electronic version of the drawing required in Section VI of this application.
