NOMINATIONS FOR COUNTY-WIDE PUBLIC HEALTH AWARDS


The Madison Department of Public Health and the Dane County Division of Public Health are seeking nominations for public health awards. The 7th annual Public Health Awards Ceremony and News Conference will be held on Monday, April 5 at 1:30 p.m. at the Warner Park Community Center.

Nominees should be individuals, businesses and organizations who enhance Madison and Dane County public health programs through advocacy, outreach, education and/or direct services. Nominees could include volunteers, public and private sector direct service providers, educators and policymakers. Environmental health, health care and health education are examples of program areas in which nominees may be involved. Employees of the Madison Department of Public Health and the Dane County Division of Public Health and past award winners are not eligible for nomination. Past nominations that have not been selected, may be nominated again. Click for a listing of Previous Award Winners.

Please take time to reflect on people and organizations that you have come in contact with that you feel have made a difference in public health. For submission of a nomination, use the Nomination Form provided.

The deadline for nominations is Friday, February 20, 2004.

Awards recipients will be notified by mid-March.
