Well Child Clinic

Services are FREE for children age 2 months - 18 years, who are residents of the City of Madison and Dane County.

Appointments Are Needed. Please call 246-4516 to set up an appointment.

3:30 - 7:00 pm

South Madison Health & Family Center-Harambee
2202 S. Park Street
1st & 3rd Thursdays of the Month
1:00 - 4:00 pm

2nd & 4th Thursdays of the Month
8:30 - 11:30 am

Physicals include: 

  • Anemia screening (hemoglobin)
  • Developmental assessment
  • Dental assessment
  • Hearing and Vision screening
  • Immunizations
  • Lead screening
  • Nutrition and health counseling
  • Physical assessment
  • TB skin tests as needed

Physicals by Public Health Nurses for:

  • Routine check-ups
  • Day Care
  • Head Start
  • School (but not WIAA sports physicals)
  • Camp

Services are especially for families without health insurance or limited insurance not covering Well Child exams and immunizations. Parents or legal guardians of children with Managed Care Medical Assistance should contact their HMO for these services.

Please bring your immunization cards.

Persons in need of interpretation or accommodations for a disability should call 246-4516 for more information.
