To apply for Metro Transit Paratransit service, it is necessary to set up an in-person assessment with Metro staff.
 

About In-Person Assessments

In-person assessments help identify the applicant's functional ability to use Metro Transit fixed-route buses. This information is used to determine whether a disability prevents use of the bus which is critical information in deciding ADA Paratransit eligibility. All assessments are conducted by a trained Paratransit professional.

The assessment consists of a physical, and/or cognitive, and/or sensory evaluation of the applicant’s functional abilities. For example, this may include evaluation of an applicant’s gait and balance, step climbing ability, bus route and landmark identification, short term memory, and attention span or other functions necessary for use of the bus.

Assessments usually take less than 30 minutes. Determinations are made within 21 days of receiving a completed application and participation in the assessment.

 


Step-by-Step Application Process

  1. Fill out a paratransit application. Applications are available at mymetrobus.com/paratransit or by calling (608) 266-4466.

  2. Set up an an-person assessment by calling (608) 266-4466. Assessments are available Tuesday through Thursday between 9:00 AM and 3:00 PM. (No holiday or evening appointments are available.)

  3. Bring completed application to your assessment, which will be conducted at Metro Transit’s administration office at the Village on Park, 2300 South Park St., Suite 109. Metro can provide transportation to and from the in-person assessment.

  4. In addition to a completed application, bring along a current form of identification, and any school or employer unlimited ride pass that you might be using to ride.
