New paratransit applicants must fill out an application, and make an appointment for an In-Person Assessment at the Metro Administration Office.

About In-Person Assessments

In-person assessments are used to help determine the applicant's functional ability to use Metro Transit fixed-route buses. All assessments are conducted by a trained Paratransit professional.

The assessment consists of a physical, and/or cognitive, and/or sensory evaluation of the applicant’s functional abilities, which include but are not limited to: gait and balance, step climbing ability, bus route and landmark identification, short term memory, and attention span.

Variables in the environment, the application, and the applicant’s ability to perform tasks required to ride fixed-route, are also considered.

This program is funded in part by the Federal Transit Administration (FTA) as authorized under 49 U.S.C. § 5310 Mobility Options of Seniors and Individuals with Disabilities Program (CFDA 20.521).


Step-by-Step Application Process

  • Step 1: View and Print the Paratransit Application, or call Metro at 608-266-4466 and we'd be happy to mail one to you.
  • Step 2: Complete the application to the best of your ability.
  • Step 3: Call 608-266-4466 to schedule your in-person assessment appointment with Metro.

Appointments can be made Monday through Friday from 9 AM to 3 PM. No holidays or evenings.

Our office is located at: 1245 E. Washington Ave. Suite 201 - Parking & accessible entrance is available behind the building off of Baldwin Street.

  • Step 4: Please remember to bring the following to your assessment:
  1. Your completed and signed application
  2. A current form of identification
  3. Any medical documentation of disability/mobility limitations
  4. Any school or employer bus pass issued to you.


Assessments usually take less than 30 minutes. Determinations are made within 21 days of receiving a completed application and assessment.

Appeal Process
