    Q~ Where can I obtain self-report forms and accident forms?
    A~ To obtain self-reporting forms, please call the Self Reporting Unit at 245-3662.
    Q~ Where can I pick up copies of accident and police reports?
    A~ For information on how to obtain copies of accident and police report forms, call 266-4072 (for accidents) or 266-4075 (for all other reports.)

    A~ Stray animal complaints are handled by the Animal Control Officers who work for the City of Madison Health Department.  To report a stray animal call the Health Department at 267-1989 or call 255-2345 for dispatch to immediate problems.

  • Curfew Ordinances in Madison
    Q~ What are the Curfew regulations in the City of Madison?
    A~ It is against city ordinance for any person under the age of 17 to be upon the streets, alleys, or public places in the City of Madison on:
    1.  Sunday - Thursday: 11:00 pm thru 4:00 am
    2.  Friday and Saturday:  12:00 midnight thru 4:00 am

    It is against city ordinance for any person under the age of 14 to be upon the streets, alleys, or public places in the City of Madison on any day after 10:00 pm.
