Dog and Cat Licensing

All City of Madison residents owning a dog or cat that is 5 months of age or older must have their pet licensed. The license year is January 1 through December 31.

Apply Online

  • Please note: To apply online you will need digital versions of or be able to scan in your dog or cat's proof of current rabies vaccination and, if applicable, proof of spaying or neutering so that they can be attached to the license application.
  • If you do not have one already, you must create an account on the City of Madison Licenses & Permits portal to fill out this application.
  • Continue to Dog Registration or Cat Registration

Renew Online

  • If you have previously applied online, or you have received a letter with account login information, login to renew your license at City of Madison Licenses & Permits. Once logged in, click the Licenses/Registrations tab or the link to Renew License or Registration. You should see your license listed and a link to Renew Application.
  • If you have not previously applied online and have not received account information, then you may apply online by following the 'Apply Online' instructions above.

Apply By Mail

  1. Dog Application Form or Cat Application form APPLICATIONS CANNOT BE TRANSMITTED BY FAX OR THROUGH INTERNET E-MAIL - THEY MUST BE MAILED ALONG WITH YOUR PAYMENT AND ANY OTHER NECESSARY DOCUMENTS. Note: The above forms are in Adobe PDF format and will require the Adobe Reader software to read & print. You can download it FREE from Adobe.
  2. Provide proof of current rabies vaccination.
  3. Provide proof of spaying or neutering if applicable.
  4. Enclose a check (made payable to the City Treasurer)
    • Regular License Fees, Cat or Dog
      • Spayed or Neutered: $15.00
      • Unspayed or Unneutered: $20.00
      • Late Fee: Dog and cat licenses are subject to a late fee if purchased on or after April 1 of the current license year. Do you have to pay a late fee? If you answer yes to all of these questions, you must pay the $5.00 late fee in addition to the regular license fee.
        • Is today on or after April 1?
        • Is your pet over 5 months of age as of today?
        • Have you had your pet for more than 30 days?
        • Have you been a City of Madison resident for more than 30 days?
    • Prorated License Fees (in effect July 1 - November 15), Puppy or Kitten
      • Is your pet eligible for a prorated license? If you answered yes to both questions, your license is the prorated amount. If you answered no to either question, your license fee is the regular amount.
        1. Is today on or after July 1?
        2. Is your pet five months of age or younger?
        3. Include an explanatory note if you have been in the City or have owned your pet for less than 30 days and are not paying the late fee.
      • Spayed/Neutered: $7.50
      • Unspayed/Unneutered: $10.00
  5. Enclose a self-addressed stamped business size envelope for the return of your documents and your new license.
