This is an official City of Madison Police Department document, which will become the official police report for this incident. If you have insurance coverage, this form will assist you in filing a claim. Please note that you must include YOUR FULL NAME and BIRTHDATE for the report to be officially filed. We will contact you if additional information is needed.

If you have a situation that does not rise to the level of needing to file a self-report for insurance purposes or if you have a simple matter (non-emergency) that you would like to report as an FYI only to the district, please go to the "Districts" tab on our website, select the appropriate district that you live in and you can contact the district directly through their individual pages with this information.
