Obtaining a Certified Birth Certificate

In order to obtain a free identification card for voting purposes, you will need to present a certified birth certificate to the DMV.  There is a fee for obtaining a certified copy of a birth certificate.

Born in Dane County

You may get your certified birth certificate from the Dane County Register of Deeds, 210 Martin Luther King Jr Blvd # 110; 608-266-4141.  http://countyofdane.com/regdeeds/default.aspx   You will need to provide:

  • $20 fee
  • Wisconsin driver’s license or Wisconsin identification card with photo
  • Current college or high school identification card with photo

OR

  • $20 fee
  • Document with your name and address (Utility bill, Wisconsin hunting or fishing license, government-issued photo identification badge, United States passport, check or bank book, health insurance card, recently signed and dated lease, or traffic ticket)

OR

  • $20 fee
  • Family member or friend with photo identification who will corroborate for you

 

Born in another Wisconsin county

You may obtain your certified birth certificate from the Register of Deeds for the county in which you were born, or from the Wisconsin Department of Health Services, 1 W. Wilson Street, Room 158.  http://www.dhs.wisconsin.gov/vitalrecords/birth.htm

If you are obtaining your certified birth certificate from the State of Wisconsin, you will need to provide:

  • $20 fee
  • One of the following:
    • Wisconsin driver’s license
    • Wisconsin photo identification card
    • Out-of-state driver’s license
    • Out-of-state photo identification card

OR

  • $20 fee
  • Two of the following:
    • Government-issued employee identification badge with photo
    • United States passport
    • Checkbook or bankbook
    • Major credit card
    • Health insurance card
    • Recently dated and signed lease
    • Utility bill
    • Traffic ticket
