File an Ethics Complaint

If you believe someone has violated the Ethics Code, you can file a complaint with the Ethics Board.

  • You must file the complaint within 12 months of when you believe the violation happened.
  • You will need to attend any meetings when the board considers your complaint.
  • You will need to make your complaint under oath.
  1. Write the complaint

    You must make your complaint in writing. You can fill out this form:

    This form is designed to help you provide all necessary information, but you are not required to use it.

    Your complaint must include:

    • Your name and address
    • The name, address, and position of the person who you believe violated the Ethics Code
    • The facts of the complaint: A detailed description of the violation, including which sections were violated
  2. Notarize the complaint

    Your complaint must be notarized before you send it. A notary must witness you sign the document and verify your identity.

    The City Clerk's Office provides notary services. The Madison Public Library also has a list of notary services.

  3. Send the complaint

    Send your complaint to:

    You can also drop your complaint off in person.
