Incident Information
Please state all the facts that support your claim, and why you believe the City of Madison is responsible.
Specify month, day, year, and time if possible.
Include a street address and additional location details if possible.
Please list names and contact information.
Police Details
Enter Report Number if a police report was filed.
For example, City of Madison Police, UW Police.
List all damages including estimates for repair costs, or attach a list below.
