The City of Madison collects information and maintains records about members of the public. The City places some of this information online where anyone with Internet access can see it. Information and records may stay on the Internet even after the City removes it from its website.

"Personally identifiable information" (PII) is unique information that can identify a person, such as a name, social security number, driver’s license number, or financial account. Technical information such as domain names and IP addresses are not considered PII.

We only collect PII that you choose to give us. You might include it when emailing us, signing up for notifications, filling out surveys, completing online forms, or paying a bill. You may choose not to contact us in one of these ways.

Some PII is protected by federal or state laws. PII not covered by those laws are treated like any other public record and may be subject to inspection and copying under the Wisconsin Public Records Laws.

Health & Safety Opt-Out

If you believe that having your PII publicly available online or released through a public records request could endanger your life, health, or safety, you can request to have it kept private. Complete the Health & Safety Opt-Out Form to make this request.

By completing this form, you are requesting that the City make a good faith effort to withhold your personally identifiable information from being publicly available on the City website due to your concerns.

The City of Madison is subject to Wisconsin Public Records Laws. The City Attorney’s Office will decide if your information can be withheld from a public records request on a case-by-case basis. If we withhold the information, requestors have the right to challenge the decision in court.

The City will notify you in writing as to whether your request has been approved or denied.
