2025 Appointment Dates
Scheduled appointments are held from 12:00pm - 3:00pm on select dates in the Madison Municipal Building, typically on the third Wednesday of the month. Appointments may also be scheduled at your worksite or another location convenient to you by contacting the plan representative directly.
