New Process to Report Workplace Injury

There is now a new, easier way to report a workers’ compensation injury.

The City of Madison is partnering with Paradigm.

If you are injured on the job, simply call the Nurse Triage Line at (844) 847-8709 to report your injury.

This will replace the old paper form we have been using. You must also report your injury to your supervisor at the time it happens.

Advantages of the Nurse Triage Line:

  • No paper report to complete
  • Available 24/7
  • Knowledgeable guidance for care, including home care
  • Connecting you with an appropriate medical facility, if necessary
  • Providing information/forms to the treating facility
  • Follow up to check how you are doing when they’ve recommended home care

Other parts of the workers’ comp process will not change. You will continue to submit a Medical Status Report each time you visit a medical provider. You will continue to complete and submit timeloss worksheets while on workers’ comp in order to get paid. It is only the initial injury report that has been simplified into a couple of contacts:

1) call the Nurse Triage Line

2) contact your supervisorPaper workers’ compensation forms will no longer be accepted as of June 1, 2022. We will be providing wallet size cards, posters, and other materials that have the Nurse Triage Line number.
