Reporting Requirements

An initial installation certification (as-built) stamped by a Professional Engineer (P.E.) registered in the state of Wisconsin shall be submitted to the City Engineer upon completion of construction.  The as-built shall be of sufficient detail to show the system was properly constructed and is functioning as designed.  It shall include a statement by the certifying P.E. and photograph(s). 

An annual report shall be submitted via email to City Stormwater Management staff. The report shall include records of all inspections and maintenance performed for the year. Inspections shall be done every calendar year, with reports being submitted by June 1 of the following year.

Maintenance Provisions

Owner shall install and maintain filter in accordance with plans approved by City Engineer and in accordance with WDNR Conservation Practice Standard #1010 Proprietary Stormwater Filtration Devices. Any alterations to approved system shall be approved by the City Engineer.

  1. An [Up-Flo Filter manufactured by Hydro International of Portland, ME containing _____ filter modules] with CPZ media mix shall be installed in the storm catch basin [location], in accordance with plans approved by the City Engineer. Any alterations to the approved Up-Flo Filter system shall be approved by the City Engineer.
  2. Owner shall maintain records of installation, inspections, cleaning, filter media replacement, and any other maintenance in accordance with Chapter 37 of the Madison General Ordinances. 
  3. Owner shall inspect and maintain unit in accordance with manufacturer's instructions and according to the following chart.

  • Regularly during first year of installation.
  • Every 6 months after the first year of installation.
Floatables/Oils Removal
  • Twice per year or as needed.
  • Following a contaminated spill in the drainage area.
Sediment Removal
  • Twice annually the first year of installation.
  • Every 6 to 12 months after the first year of installation, depending on past experience.
  • When sediment deposits reach 12" in depth.
  • Following a contaminated spill in the drainage area.
Media Pack Replacement
  • Minimum once per year.
  • Replacement is required any time inspection reveals that the high-water level indicator has been activated after two consecutive storms and the subsequent weighing of the Media Bags shows a wet weight greater than 40 lbs.
  • Following a contaminated spill in the drainage area.
Drain Down Filter Replacement
  • Once per year with Media Pack replacement
  • Replacement is required anytime inspection reveals that the water level inside the vessel has not reached a level equal with the base of the Filter Modules approximately 36 hours after a 1-inch rainfall.
