Request a City Photo ID
To request a photo ID, contact Human Resources at hr@cityofmadison.com. HR staff will follow up with information about timeline and cost (if needed).
For security purposes, employees cannot request their own IDs. The following people (in your department) can request an ID:
- supervisor or manager
- department or division head
- authorized administrative person
