New CDL Holder
- Take the 3 permit tests- General Knowledge, Air Brakes, and Passenger
- Choose Tier 4 for the license
- K Restriction will be placed on the license, and no Fed-Med is required
Have a CDL
No passenger endorsement
- Take the passenger written test / pass test
- If you have a Tier that requires a Fed-Med, you can keep it or forfeit it and change your Tier.
- Form 3230 at DMV, Tier of operation.
- Tier 4 and K restriction does not require a Fed-Med
With Fed Med Requirement (similar to above)
- Keep or forfeit your Tier, your choice, change tier with MV 3230
- If you keep the Fed Med Required Tier, Fed-Med renewal will be at the employee's cost
- If a Fed Med is required for a medical reason, it must have a valid Fed Med with a license
