2024 Older Adult Services RFP (Closed)
Purpose
The Community Development Division (CDD) of the City of Madison seeks to create the necessary conditions for Madison residents to realize their full potential through the building of human capital and economic development. Encompassed in the goal of Shared Prosperity through Building Human Capital is the assurance that older adults are prepared for lifelong success.
This application process is intended for organizations who have contracts managed by CDD in 2024 for Older Adult Service programs; or are new to City of Madison funding for Older Adult Services.
The Older Adult Services funding process for contracts beginning January 2025 is open to organizations who wish to apply for City funding for;
- Case Management Services,
- Culturally Relevant Services,
- Independent Living Support Services,
- Outreach, Information, and Referral Services.

Attend a Workshop
Applicants are required to attend an RFP workshop or schedule a conversation with City staff Yolanda Shelton-Morris or Garrett Tusler about their proposals by June 5th, 2024, at 11:00am-1:00pm to be eligible to apply.

Submit Letter of Intent
Applicants wishing to be considered for funding through this RFP process must submit a Letter of Intent form by Monday, June 17th, 2024, 4:30pm to CDDapplications@cityofmadison.com.

Applications must be received by 4:30pm on July 1st, 2024.
Proposals must be submitted to: CDDapplications@cityofmadison.com
All proposals must be submitted electronically. Please submit all application materials by the program proposal. In other words, if your agency plans to apply for three different programs, we would expect to see three separate email submissions including all of the required materials listed below. This will facilitate the review process.
Format for File Names
Please follow the file naming format provided below for all submitted materials:
[Program Name]_[Your Agency’s name]_[Program Type]_[Document Type]_[Applicant Type]

Please include only the required submittals specified below:
- One Application Part 1 – Organization Narrative form
- One Application Part 2 – Program Narrative Application form
- One Budget Workbook, which includes all organization and program information.
- One (or more) Budget Narrative document – Joint/Multi-agency applications only
- Letters of Commitment – Agencies listing a partner/collaborator in addition to any joint/partner applicant (if applicable) for their program should include a letter of commitment/support from the listed agency highlighting the ways in which the agency will support the program.
- Fiscal Agent Form -- Complete the form if applicable
Applications time-stamped received after 4:30pm on July 1st, 2024, will not be considered. No exceptions!
You will receive a confirmation email once you have submitted your application(s) if submitted before 3:00pm on July 1st. These email notifications are not auto-generated, so allow some time for a person to receive and process your application. If you do not receive a notification within 2 hours, it means that your application did not get delivered.
